Eligibility Criteria :

  1. Applicant should be either a Proprietorship/Private/Public or Govt. entity.
  2. Applicant should be minimum 3 years old.
  3. In case applicant have less than 3 year of experience than the original Manufacturer Authorized Letter from the Manufacturer should be sent to
  4. Applicant should have minimum 5 government clients during the last 5 years.
  5. Applicant should either be Manufacturer or Authorized Distributors/Stockist.
  6. Applicant should have valid Satutory Tax Registration.
  7. Applicant is requested to pay the registration fees of ₹ 1000/- in form of demand draft (DD) to be issued in favour of "Translational Health Science and Technology Institute (THSTI)" payable at Faridabad.
  8. Canvassing in any form will be a disqualification.

Following is the procedure to fill online application :

  1. Before filling online application, do keep the following documents ready with you:
    1. Scaned copy of Company registration certificate or Scaned copy of Manufacturer Authorized Letter from the Manufacturer (in case you have less than 3 year experience.
    2. Scaned copy of Purchase orders or Rate Contract for all 5 govt. clients (individually) in last 5 years in PDF format only.
    3. Scaned copy of CA certificate indicating Annual Turnover.
    4. Scaned copy of GST registration certificate & Copy of PAN card.
    5. Application contains two (3) Parts. Part I - for vendor basic details , Part II - the description of products and Part III - Online Payment.
    6. You have to register with company's valid email id.
    7. After successfully registration, you will receive an auto-generated email containing a reference number & login details on registered email id . Please note down the reference number for any correspondence.
    8. On receipt of login details, vendor registration application can be filled.
    9. Before submission of form Part I or Part II can be changed.
    10. The application will be considered complete on receipt of the non-refundable online payment of Rs. 1000/-. Upon successful transaction, the applicants are requested to update the transaction details in the portal and submit.
    11. After successfully submission, take a print of the registration application.
    12. Please sign the application at the appropriate place & send the signed application along with DD, self attested copy of certificates etc. as indicated in application form. So as to reach at the below address:
